Restored Life Counseling (“we,” “us,” “our”) is a nonprofit counseling practice and, for clients receiving clinical services, a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). This policy explains what information we collect on this website, how we protect your health information, and your rights.
Two kinds of information, two kinds of protection
Website information is what any website collects: pages visited, general analytics, and anything you voluntarily submit through forms. Protected Health Information (PHI) is information about your health care — including the fact that you are (or asked about becoming) a client. PHI receives HIPAA’s stronger protections, described below.
How we protect your health information (HIPAA)
- Intake, contact, and screening forms run on JotForm’s HIPAA-compliant platform under a Business Associate Agreement (BAA). We do not collect health information through this website itself.
- No advertising pixels or third-party trackers run on our health-related, intake, scheduling, or survey pages.
- Your survey results are not stored by this website. The free Personal Challenges Survey is anonymous; results are displayed to you and not retained on our web server.
- Clinical records are kept in HIPAA-compliant electronic health record systems, not on this website.
- Business Associate Agreements are in place with vendors that handle PHI on our behalf.
How we use and disclose PHI
For clients, we use and disclose PHI only for treatment, payment, and health care operations (for example: coordinating your care within our team, billing your insurance, and quality improvement), or as required by law. Disclosures required by law include situations involving imminent danger to yourself or others, suspected abuse or neglect of a child or vulnerable adult, and court orders. We do not sell your information — ever. Any other use requires your written authorization, which you may revoke at any time. Clients receive our full Notice of Privacy Practices at intake.
Your rights
- Request access to and copies of your records
- Request corrections to your records
- Request restrictions on certain uses and disclosures
- Request confidential communications (for example, a specific phone number or address)
- Receive an accounting of certain disclosures
- File a complaint with us or with the U.S. Department of Health & Human Services Office for Civil Rights — we will never retaliate against you for filing a complaint
If a breach occurs
If a breach of unsecured PHI affects you, we will notify you without unreasonable delay and no later than 60 days after discovery, as HIPAA requires, and we will notify the Department of Health & Human Services and, where required, other authorities.
Website analytics and cookies
On general marketing pages (like our homepage and blog), we use privacy-respecting analytics to understand how visitors find and use the site. These tools are configured not to run on health-related, intake, scheduling, or survey pages. You can decline non-essential cookies via your browser settings or our consent banner where shown. Emails you receive from us (if you opt in) include an unsubscribe link, and we honor opt-outs promptly.
Children
This website is not directed to children under 13, and we do not knowingly collect personal information from children online. Counseling services for minors are arranged by a parent or guardian.
