Life is complicated. Mental health is, too. Many people feel depressed or anxious, but those may be symptoms, not causes — so good mental-health screening looks at several things at once, not just one thing at a time.
The free, anonymous Personal Challenges Survey below helps answer that chicken-or-egg question by asking about several issues at once: depression, anxiety, safety concerns, addictions, relationship skills, and relationship satisfaction. It takes about 15 minutes to complete.

A starting point, not a diagnosis
This survey is an information tool to help guide you toward better emotional and relational health. It is not a professional diagnosis. If your results concern you — or you already know you want help — our clinical team is ready.
The PCS as a research instrument
The Personal Challenges Survey is more than a screening tool — Restored Life Counseling is developing it into a validated research instrument so it can serve people and clinicians more effectively.
We are establishing a formal validation study under the oversight of an Institutional Review Board (IRB) — an independent committee that protects the rights and welfare of research participants. That study is currently pending IRB formation and approval; until it is approved, your responses are used only to generate your own results, not for research.
Once the study is approved, de-identified responses — carrying no name, email, or identifying information — may be used to validate and improve the instrument for broader clinical use. The survey is, and will remain, free, anonymous, and secure, and it is never a substitute for a professional diagnosis.
